If a tiny piece of the plaque that’s lining an artery comes loose- it can float around in your bloodstream until it gets jammed in one of tiny arteries/ capillaries in the brain. This leads to the part of the brain supplied by that artery dying because it’s got no blood supply. This is called a stroke.
To prevent this, the operation involves exposing the artery, clamping either side of the plaque (sometimes with a bypass (called a shunt) to ensure that blood gets past the isolated section of artery) and cleaning the plaque out with an instrument (for an aorta (the big artery from your heart)- we use a (sterile) spoon!).
We use loads of heparinised saline/ hartmanns’, to prevent clots forming.
If the operation’s on a carotid artery (in your neck), sometimes the patient remains awake, so we can judge how much blood’s getting through from the other side (by talking to the patient).
Sometimes the plaque extends into the skill; this is inoperable as we can’t clamp inside solid bone. Clamping below or onto an area of plaque is not a good idea because it can cause part of the plaque to become dislodged, leading to a stroke. In cases like this- we stop the operation.

In your situation, it usually takes a lifestyle overhaul to get over the problems, along with medication to keep the cholesterol under control. Often, just changing you diet, and walking around the block for 20 minutes a day can make a huge difference. If you are already watching your diet and exercising, then the docs might be able to prescribe medication.
